I phoned my GP surgery and they have now received an evidence form for my GP to complete. The paper based assessment the DWP guy said today looks like it is coming through, one screen is implying it has been submitted but the other screen not, the DWP guy said this implys it is on its way and will prob show up properly by monday. i have a phone call booked with my GP for monday regarding a health problem so am wondering if I should discuss the ATOS form she has been sent, My main concern is the fact that past tests did not show damage, my medical notes read NAD as in 'nothing abnormal found' which was in my evidence sent to ATOS but according to my consultant these tests do not always show damage. There is a letter from the consultant saying that the tests are not conclusive but I did not send that to ATOS. Should I bring this to my doctors attention for when she fills the form in? Do you know if GPs use medical notes alone for reference when they complete the ATOS form or do they look at hospital letters? I am concerned that my GP may write about tests not revealing any problems/damage without explaining the essence of the other consultant letter saying the tests are not conclusive. Or should I just say nothing?

Hi Cruella,

I don't know what information GPs use when filling in the forms. I suspect it varies from a full review of the patient's records to a cursory glance depending on the doctor concerned.

As you have aa phone call with the GP booked for Monday, this would seem to be the perfect opportunity to discuss the form that is on its way.

The bit that I sent in my evidence said tests and NAD, quite subtle and amid a page of lots of useful evidence which is why I sent it

It should only take a day or two unless the report has to go through audit when it make a week or more.

oh dam, think there is a problem with mine then. It has been been more than a week and i think it has already been audited. DWP when i phone them keep saying give it a bit longer but also say it is being chased up. Is it possible do you think for the report to get lost ?
