Is it unheard of for a claimant to ask and successfully obtain a letter from a GP recommending a telephone rather than face to face home assessment even

It's not impossible but I think it unlikely!
The Assessment Provider has a legal responsibility to seek sufficient evidence in order to make a proper recommendation on how a claimant meets the PIP criteria. For a phone assessment, there must be sufficient information in your PIP2 and evidence attached to make this a realistic possibility. A letter from your GP is not going to carry much weight unless you have met the information requirement first.

Hi, I finally managed to get some evidence from my private counsellor.
I have copies of notes she has made since October. The problem is her notes are so scanty and vague. I have been seeing her primarily for my contamination phobia which hugely impacts on my life but her notes do not support his. Every session we have had we have discussed my contamination phobia but her notes refer to anxiety and that is only once. Now I am feeling that these notes may even be bad evidence because surely the DWP will look through them and think it strange that there is no specific mention of contamination phobia. I am so frustrated.

Sometimes no evidence can be better than bad evidence.

Hi. My counsellor has offered to write an accompanying letter to go with her notes and in the letter she says she can explain how my anxiety is related to a contamination phobia that dominates my life. She is asking me where my notes and application will be sent to so she knows whose name and address to use when putting together a letter. I shall definitely need this letter for PIP evidence so I should ask her to use the PIP address at the top of the letter.?? (she will pass his letter to me).

They can use the address of the DWP office dealing with your claim and "for whom it may concern".
The letter really need to be attached to your PIP2 form and you need to write on your full name and National Insurance Number at the top.
