DLA allowed an award to be made because of a general need for care, many claimants with epilepsy qualified under this, PIP has no equivalent, so you will need to argue that you need supervision for activities such as Preparing Food and Washing and Bathing, etc.
I would certainly mention the shower, but I don't think it is going to help you score points unless you have other problems.
As to Managing Toilet Needs, I suspect that you struggle with the "majority of days" rule, so unless you are having fits at least four days a week and soiling yourself as a result, a Decision Manager may take the view that this is not the majority of the time, but it is certainly worth your explaining your problems, because if you don't you certainly won't score points.
The PIP Descriptors make no reference to your having to be awake and there is no Case Law in this area.

The Going Out activity, where the changes are, only considers cognitive, mental health and sensory issues, the DWP consider Epilepsy to be a physical condition. The only way to score would be to show that the Epilepsy causes anxiety leading to "overwhelming psychological distress" that would prevent you from going out alone.
