My son receives ESA and is in the Support Group. As his Appointee, I have just received ESA50 04/15 form as the DWP is reviewing his award of ESA.

1) I have downloaded the form unto my computer. When giving detailed information in the boxes provided with each question to explain his difficulties, there is very little room to give a full explanation. The text font does not reduce to allow more space (it did on the original claim form 4 years ago). Has anyone else found this problem and how was it overcome other than by using separate sheets of paper.

2) My son's medical conditions and disabilities have not changed. Is there a way of just providing an up to date medical report from his GP instead of completing the ESA50 Form?

3) I note the ESA50 Form is now called 'Capability for Work Questionnaire. It was originally called Limited Capability for Work Questionnaire. Is there any significance in the change of title? Do I have the correct form?

To answer your questions;

1. The comment boxes are fixed, those who are of a cynical nature might think it was to limit the response that a claimant can make. It's perfectly acceptable to include additional pages, just make sure that the all have your son's full name and National Insurance Number, I would also number them.

2. I'm afraid not!

3. Yes it's the correct version. The form has had a number of cosmetic changes over the years, although it is an official document and is part of the ESA assessment process, it does not in fact contain the assessment criteria as defined in law, thee are included in the guides.

To add to Gordon's excellent advice -

The assessor will have no prior knowledge of your son, including whether he is currently in receipt of ESA. That is why you must approach every reassessment in the same way as the original claim, filling out the ESA50 in detail and submitting supporting evidence.

Thanks for the advice.

I am of course using your excellent Guides again which were a fantastic help with the original very successful ESA claim. In particular, your Guide for claims on Mental Health Grounds dated January 2016. I am wondering if the ESA50 form version 04/15 sent to me has been further revised as your Guide refers to (for example) in Descriptor 11. Learning Tasks to 11(a), 11(b) and 11(c). There is no mention of 11(a), (b) or (c) on the form and the questions on the form are different to those in your Guide. Please advise.

I did say that the form does not include the legally defined criteria against which claimants are assessed, at best it is a poor précis of these.
