There are two assessment reports, a PA4 for a face to face and a PA3 if the assessment is done on paper only. The Assessor will include their scoring recommendations in these reports. There is unlikely to be any other documentation that you should not have already received.

You make the request over the phone or in writing to the DWP office dealing with your claim.

It usually takes 7-10 days for the report to arrive.

Thank you for reply Gordon, shall ring them first thing in morn. Am presuming I ring same place as I rang when I rang to ask for the two week extension, is that correct?

Rang them first thing. Still shaking, thought I was gonna throw up. Will take rest o day to calm down. Was told it may take couple o weeks to arrive. Asked if I should write to confirm my request and was told 'no', but as they're not to be trusted think I'll write anyway. :)

Hi Porthole,

Well, having just sorted out PIP, today I get forms for ESA reassessment. I'm a wreck, this is so stressful and difficult. They make the forms so hard to understand!

Just wondering if you've had any news yet?
