New PIP Assessment form from INDEFINATE DLA award

I have been given an extension to fill my new PIP form out after being on DLA for years with 22 illnesses & my health getting worse. The extension has run out, as I am having panic attacks every time I try & fill the form in...I'm terrified of doing it wrong & losing everything (I am paying for my own medical treatment with my benefits, as it's not available on the NHS..so A LOT is at stake...basically my life). I am SO SO sick!! The 80 pages of the guide overwhelms me as my brain does not work well & the stress triggers my suicidal depression from my Bipolar & PTSD. Does anyone have any advice? Do I copy off the form? What answers do I write? Do I keep it simple etc? The form is great but my brain cannot take it all in. HELP!!!!!
THE ORGANISATIONS RECOMMENDED TO HELP EG: C.A.B ARE USELESS. Any advice about filling the form in would be appreciated & on what you did. Thank you. xx

Forgive me if I am blunt. if you don't get the PIP2 form sent back then the most likely outcome is that your PIP claim will be closed and your DLA claim four weeks later.

You say that the extension has already run out, so the first thing you need to do is contact the PIP Claim line and ask for another one.

As to filling in the form, follow the formula in the guide and just write a short sentence in answer, do through all of the questions and then go back to the beginning and try and change each sentence in to a short paragraph. Keep trying to expand the amount of information you are supplying each time.

Take regular breaks so that it does not seem to imposing, if you feel you need to stop try writing just one more sentence / paragraph before you do.

Make use of the PIP Claim guide to help you understand what you need to explain, in particular try and use the same language as the activities do and remember to include information about your ability to do things "reliably" and for the "majority" of days.

I know you can do this, the biggest hurdle is starting, once you have I am sure that you will find it easier to continue.
