Hi new to here need help ...65 DLA to PIP
Couldn't even fine where to write message ,the pills I am on our making me blurred eyes and wobbly etc . I am on DLA indefinite need to complete PIP form and send it back by 19 oct. I have been trying to do a diary not sure if I am doing even that right ,trouble is I have so many things wrong with me I am in a complete muddle a few of the things are :work accident years ago ,meniere disease , copd , arthritis in back ,diverticulitis,at present on strong medication as well as usual pills

From your post you have received an invitation to claim PIP.

I would start by having a look at PIP Claim guide on the following link.

If you find that you are running out of time to complete the form then you can request an extension of 2 weeks by phoning the same number as you did to start your claim, I would phone about a week before it is due back to ensure that you still have time if the request is refused, although I would not expect this to happen.

PIP question 13/14 still after looking at ur samples don't know what to write as so much wrong with me , since mid august when my back seized up doctor says trapped nerve as well on pregablin and other pills vision blurred ,wobbly etc so do I fill form in as I am now or b4 this last problem happen don't know when or how much better I will get .

Sorry Gordon ,meant to say I was on DLA indefinitely so have sone old letters few new hospitals ones ,never thought I would need them again

Hi again , can't understand what they mean about satellite nav in Q 13 If I say I use sat nav does it go towards getting or will it go against me ?

You need to complete the form as you are.

The Going Out activity is aimed at claimants with mental health, cognitive or sensory issues, the Moving Around activity is for those with physical problems.
