It's not really a transfer process, your ESA will stop and your State Pension will start.
Any payments that are enabled by your ESA will stop, for example Premiums etc.
The State Pension is not means tested so any additional benefits you might have received if you were receiving any Income Related ESA will also stop, however, you may be able to claim Pension Credits as a replacement and this will re-enable them.
Be aware that the State Pension is liable to tax.
