Hi good evening I have a question been trying to find a topic about Medical reports form explained but cant seem to find any..... my son had a ESA medical on 12th September 16 and has been put in the support group this is good I think At the medical did ask for a copy but not get the medical report with this letter.

So I had to phone wembley and ask for it, it came this a morning but it is only 4 back to back pages with 1 - 39 questions NO POINTS we were wondering is this normal I thought you were awarded points system.
He has personality disorders
On the bottom of each page it says (RESTRICTED - MEDICAL)

We only wanted to know because it has been said that my son might be able to get DLA on mental health grounds alone. Would be nice to know what point if any he was given

If you can help in anyway with any information i would be very grateful for your help

A full ESA assessment report can be upwards of 30 pages. Two possibilities.

They've sent the wrong report, you need to ask for the ESA85, although I would have expected your requesting the "assessment report" to have produced the same result.

The report contains "harmful information" and you have only been sent the entries that are not restricted. This means that some or all of the content is considered to be at risk of causing harm to the claimant if they were to become aware of it. This would be a very unusual situation but is not impossible. Unless you are your son's legal appointee they may be unwilling to provide the information, it normally requires a doctor's permission to be released.

HiGordon thanks for the reply...... I asked them at the medical if I could have a copy of the medical information, then the other day I asked for the same.

I just had a look in the top left corner it does indeed say ESA85 and the medical report form the last page says top line LCW or LCW and LCWRA information

Then 4 paragraphs then turn the page and it says DECLARTION then Heathcare Professionsls Name i`m still little confused no points ? Thank You Gordon

It sounds like they have only sent part of the report. The ESA85 will not contain a list of the points but the assessor should indicate which Descriptor they believe that you son has met, this is done via a 2 or 3 letter code. The first one or two letter refer to the activity and the last letter to the Descriptor, so "Wa" is for the Mobility activity and Descriptor (a) which scores 15 points.

The codes are covered in the WCA Handbook, there's a copy in the ESA section and you can cross-reference to ESA Claim guide for the points.

hi Gordon
Just got off the phone on a 45 minute wait in Q After a long look through the system she found the medical report she is now sending it in the post tonight so lets hope it has what we are looking for.
Then the next step is having a read to see if our son in intilted to PIP`s i will look up the PIP`s form online io think i might find it there
