ESA questionnaire multiple health conditions

I need some advice regarding filling the ESA questionnaire (re-assestment)
my old ESA claim was based on 2 mental conditions only, but it's been 3 years since my last assessment and during these three years I was diagnosed with 3 physical conditions.

I've received the new ESA forms now.

so I was wondering how can I complete the questionnaire with all these different conditions.
their different effects of different tasks and their similarities etc
I've read all pages of the guides.

for example to explain the difficulties I have with tasks,should I explain the problems caused by every condition separately ? this way I will be repeating myself a lot and I'm not sure that's how I should fill the forms?

or should I just write one paragraph mentioning the effects of all conditions as a whole on that task.

sorry I know it's a bit difficult to explain but any advice on how to complete the forms for multiple conditions will help.

First, you can only score points for the physical activities (questions 1-10) for an underlying physical condition and for the mental health ones (11-17) for an MH problem.

Where multiple conditions cause problems with a particular activity then I would answer just once, but if they affect different aspects then I would reply for each case.

So as an example if one condition causes you problems with starting tasks and another with finishing them then write about both separately, but if it is one condition or starting and finishing is effected by the same multiple conditions then just cover it once.
