PIP dairies was created by tyro
I have a friend who has been asked by the DWP to claim PIP for the first time.At present he is in receipt of DLA mobility and care components,awarded at First Tier Tribuneral.The care component was awarded as he was seen to be at risk of self neglect.
He has long term mental health issues.He suffers from depression and has problems with insomnia,self motivation,concentration,organising and seeing through everyday tasks,social interaction,going out in publc,panic attacks and suicidal thoughts
I have phoned the DWP on his behalf and supplied the information they requested.I was told he will be sent a PIP2 form within the next two weeks.I will be helpling him to fill this in and I also understand that it will be helpful to the claim if a PIP Diary detailing an average day is also submitted.I would also be doing this for him.
Given that he would not be capable of doing this by by own means,if this is submitted would the DWP conversely take this a negation of the problems he has with self motivation
,concentration,organising and seeing through everyday tasks?

I think there is a risk that they would, especially if he requires prompting to perform activities such as Washing and Bathing or Dressing and Undressing.
Do you see him daily, it might be possible to complete the diary retrospectively. As an alternative you could create a list of daily tasks and mark him each day on what he has achieved.

ideally he would need help on a daily basis,but I am unable to visit and help him that often.How detailed should the dairy be? Just an example of a typical day would be enough? Could I add a note to the PIP2 form saying that the dairy was filled out FOR Him? rather than BY HIM?

You can do a typical day as a separate item from a diary, as this really needs to be at least a weeks' worth and longer of possible.
Definitely explain who has completed what or they will assume that he has done it, even if it is his words that you are writing down.

Thanks for the advise.
Regarding the PIP2.I have already told the DWP on the phone that I will be filling in the form for him.On the PIP2 form example that I have there doesn't appear to be a section on it that asks if it has been filled in by somebody else on the claimants behalf.
Should I state that and sign my name in the additional information section,or is that covered in the information I have already supplied by phone?

There's a section at the back for additional information, you can put it there.
