Hi, I've had a search of the forum and can't find the answer to this - but apologies if I've missed it due to my search terms

I'm filling in my PIP form (currently on DLA) and want to point out that I have a scan planned to examine a lump in my armpit to make sure whatever it is is beingn and also a camera bowel test to help my consultant determine my diagnosis

I'm wondering whether to put this in the treatment section as the result of either or both could result in further treatment/new meds - or - just in the additional info at the end? Any thoughts?

Also - do I really need to fill the form in by hand? My hands get so painful when writing for any length of time.

Hi Stefanie,

You can include the information in either place - or even both! Whatever seems most logical to you.

No, you do not have to complete the whole form by hand. The important thing is that you return the same form that you were sent, as it is marked with a unique barcode which identifies the claimant.

What many people do is answer each question on the computer and print it out, firmly attaching all additional sheets to the back of the PIP2 form.

If you do this, you need to be sure that you have clearly identified which question you are answering. You should also write in the text box on the from something like 'see additional sheet page 3'.

Each additional page should have your NI Number at the top and the page number in the bottom right corner. We suggest you use the format 'x of y' so that it is easy to see if something has been lost from the back of the pile.

Hello!

I have gone through the form ticking the appropriate tick boxes, and in the "any other info" section written "please see typed sheet page [_], then written looooong entries for the relevant boxes in Word, then when it's finished I'll put which page each answer is on.

Great idea re. typing it and then attaching at back - sounds so much easier.
