Pip ass tomorrow, haven't seen form

Hi folks, am panicking about the assessment tomorrow, i am a full time wheelchair user due to chronic pain caused by an accident 20 odd yrs ago, but also have severe recurrent depression.
A Mental health support worker filled out the form for me & will be coming with me tomorrow, but i was encouraged to sign the form before it was completed & despite asking numerous times i still haven't seen the completed form.

So i guess my question is, how much trouble am i in re assessment & do i admit that i don't know what was written. And please if you know of a problem that i haven't thought of, please tell me.

We've seen this a lot recently with welfare advisors completing the form and not providing the claimant with a copy, it's unprofessional!

I don't think you should tell the assessor that you have not seen the form, doing so will severely undermine the credibility of it, whether it is a true reflection of your problems or less so.

Have a look at the PIP Claim guide it has a section on the assessment and the questions you are likely to be asked.

Thanks Gordan for your advice, much appreciated.

With the old black dog hanging around & my serious anxiety kicking off big time i am worried that by the time i get in the room, i will be virtually unable to explain things & answer any questions (imagine a person rocking back & forth, in a wheelchair, staring at the floor & unable to do any eye contact) & that will be me, how can i pass (Dla H rate both, swap to Pip) if i cannot speak, sorry have looked through all the stuff & couldn't find any info on mental health.
