From your description I think that the letter is a standard one as I am guessing that your appeal request was late. I would be surprised if the DWP object but it will depend on the reasons you supplied as to why it was late.

Even if they do it does not mean that your appeal will be refused, you can are entitled to answer the DWP's reasons for objecting.

FH - Functional History
IO - Informal Observations
HOC Variability - Can't find a specific definition for this, "hoc" usually means “for this” or "for this situation.", so the phrase as a whole would depend on the context.

"It is likely" is just a way of saying "it's my opinion".
