Hi everyone,
Just when I thought things couldn't be more stressful. I'm bedridden and always have welfare rights fill in my forms for me as I can't write.

I'm on ESA support group, and high rate DLA, care and mobility.

The guy came to see me. I gave him a copy of my old ESA form to help him fill it in telling him there's no change etc.

And he says that he'd tell them that there's been no change.

What he's actually done is fill in the first page of my booklet and then just enclosed a copy of my old ESA form from 2 years ago! With copies of new health reports.

I'm absolutely mortified. In effect he's sent in a blank book which is they tear off the old ESA form is literally just a blank book!

Is there ANY chance at all that he hasn't completely destroyed me? Any failiure with the ESA will lead to a change in circumstances which will scrap my DLA and lead to my money stopping from that too and a PIP form.

Is it safe to say i'm completely screwed?

It's certainly not what I would have done but I don't think that all is lost.

Maximus may accept it as it is. Did he include a covering letter explaining what they had done?

You still have the option of submitting a replacement ESA50, you can use the electronic version of the form, assuming that you are fit enough to complete it. See

Using The PDF Version Of The ESA50 Questionnaire

You can contact the Welfare Rights office and ask to speak to a manager and raise the issue and ask them to rectify the issue.

He says he's been doing this for msny years and to trust him and not to worry?

I'm terrified to contact his management because i don't know how the problem can be rectified if the forms have already been sent

It's your claim and only you lose if it goes wrong, I see no problem with you contacting his manager, if they agree with the way he is managing the claim then you can maybe rest more easy.

But I understand if you do not want to rock the boat any further.

I've taken the middle ground snd instead asked the main point of contact at welfare rights for advice without asking for a manager. Explaining what has happened. Asking to remain anonymous. My deadline is on Friday so i don't know what if anything could be done in the time given.
