Dear B&W

I have had a few more spells in hospital again so have not been able to look at updating my notes in readiness for the PIP2. However, I have now been reading your site again and now think I'll be receiving the AR1 form instead.
Could you please help with the following:

Where it asks ' what has happened ' & ' how it has (in my case) made things harder' is this just what I would have put on the old style form, only now split into two separate questions. I have multiple illnesses/ disabilities one chronic, no cure which can only get progressively worse. At my last assessment I received high rate d/living and mobility. Since then things have got slowly harder with other new things I now also have to deal with.

I'm just not sure what sort of information and how much detail should be going into each section.

Hope this makes sense.

We don't have enough feedback from members who have used the form to give definitive advice on how to complete the new form.

One thing we have seen is that where there have been changes, better or worse it seems important to give a detailed explanation of the changes.

You mention that you have been in hospital, I think it is worth you spending some time on this, the DWP seem to assume any medical treatment always result in an improvement in the claimants conditions, don't let them make assumptions.

Thank you for your reply Gordon.

I didn't realise the DWP might assume my admittances to hospital would result in an improvement in my condition. The reason I'm in hospital often is because my illness gets to the point where it can't be managed at home/in the community so intervention is needed. All this does is put me back at the point I was before, there is no cure for it, I never get 'better '.

Would I be best detailing why I went in, the treatments I received etc with regard to each activity if it's relevant, or am I best writing as a ' Further Information' and attaching to the form.

Sorry, I haven't made myself very clear.

Should I be writing about my hospital treatments within the the actual AR1 form, under each heading or just as separate piece of information on a plain piece of paper.

It really depends on the reasons you were in hospital and what treatment you had, so if it was relevant to a particular activity then it would make sense to include details in your response to that activity, for example; if you had some treatments on your hands then this would likely be relevant to the Preparing and Cooking Food activity (and others), but if it was general or not relevant to the PIP activities then include it at the end.
