Hi I've been on dla and incapacity since 2001, my dla is indefinitely but guess at some point I will have to go through changes with that.

In 2013 I had to reapply for the esa, I had to fill the esa 50 form in and was down for a medical straight away . Be time I'd sent the form off a few weeks later I got a letter to say I didn't have to attend the medical ,due to my doctors report . I was put in the support group ,I thought that was the end of it all over with.

I have now received another form to fill out, i have been waiting to see citizen advice which I saw yesterday . My doctor told me a lady came into the surgery , I booked a appointment told the lady who booked it for me what it was regarding .

Went yesterday to be told from the cab lady she didn't have time to fill it all out ,now putting me into a panic as it needs to be back by 2nd December.

She filled a few answers in for me and then said oh you can fill the rest out yourself . The part I'm unsure on is the yes, no, or unsure and how they word things I've done the test you have on line and it has come out saying how did you assess yourself ?

Did you assess yourself as being eligible for the support group on the grounds of;
a)exemption;NO
B)Descriptors: YES
C)Exceptional circumstances YES
D)Hospital treatment YES

WHICH GROUP WOULD YOU BE PLACED IN?
Based on your self-assessment: 'you will be placed in the support group exceptional circumstances. Because of your physical or mental health condition or learning disability,there would be a substantial risk to the mental or physical health of any person, including you,if you were found not to have limited capability for work-related activity.

Work related activity group, POINTS you assessed yourself as being eligible for the work-related activity group because you scored 15 or more points.

You assessed yourself as scoring a total of 120 points for the following etc etc.

I'm I ok with the points to stay on the support group on the above.

I'm waiting to have another operation to have a nerve root stimulator fitted but no guarantees whether it will help or not .

I would recommend that you use our ESA Claim guides on the following link

ESA has very specific criteria and you will need to show that you meet these to receive an award. The guides explain this criteria and offer an approach to answering the questions on the form.

Hi dawny,

Good luck with the scs, have you been waiting long for it? There is a very good facebook group that will give you lots of information and support if you want it.

Im struggling filling the form out if im honest, it has to be back by the 2nd dec , im ok answering the questions its the no , yes ,varies im unsure on which one to tick.

The "yes" or "no" indicates whether you can complete the activity or not, "it varies" would be used if your conditions are fluctuating and sometimes you can do the activity and at other times you cannot, you need the circumstances when you can and can't and in particular how often or for how long you are effected in that way, you should be marked on how you are or the majority of the time.
