esa form was created by brian grieveson

i was medicaly discharged from the army in 1978. had more ops than i care to think of since then.i have forgot how many ops i have had. my question is.how much of my medical history have i got to tell them.i could write a book.I am on 90% war disablement pension surely they cannot expect me to rememder all of my ops and mental health..
