The first thing you need to do is request a copy of the assessment report by contacting the DWP office that is dealing with his claim, the address will be on the Decision Letter.

Re the GP visit. As a Decision has been made the DWP will not contact his GP about his problems now, even if you go on to challenge the Decision. I have to warn you that it may of been convenient for his GP to expect the DWP to contact him for more information and it is possible that they will either continue to assume the position that the DWP will contact them or simply refuse to provide a letter of support.

But in the hope that they will provide assistance, they need to provide confirmation of the problems your father has, however, if they have not seen him recently this may be an issue. That said there are certainly areas that they should be able to confirm; you mention the eczema, if the GP has prescribed the creams then they should be able to comment on the necessity of applying the cream and the potential effects of not doing so.

They should also be able to, with a short examination be able to comment on your father's shoulder/back problems and whether they can dress their upper body.

You don't need to rush but a request for a Mandatory Reconsideration must be received by the DWP within one month of the date on the Decision Letter. Have a look at the PIP MR & Appeal guide for how to request an MR and there are template letters that you can use to make the request.

Once you have the assessment report, go through it with the PIP2 you complete and the PIP Claim guide to make sure that you have shown that he meets the criteria to score points. Pay particular attention to the terms used in the Descriptors, you need to bring his limitations to the Descriptors not the other way around. Some of the Descriptors have multiple elements, where this is the case you need to show he meets each of these to score points. Also make sure that you have used "reliably" and "on the majority of days" wherever applicable.

Re: GP visit
Assuming the GP is willing to assist ....
What information, if any, should be physically passed on to the GP? Should I just make a list of the points you raised and just discuss it with Dad's GP during the appointment (relying on him to note down where his input is needed)? Or should that list of points (that I take in) be left with the GP? Like I said before I don't want to do anything to jeopardise anything.

I presume that I could also get the GP to make reference to Dad's memory problems too (as detailed in paragraph 10 of the original post?

We are still waiting to receive the assessor's report. It was requested on Wed 30/11 (the day we received the decision letter dated 25/11). When it was requested, my sister was asked if it was needed for MR. She told my sister that we have a month from the date of the decision letter (as you explained) but supporting evidence can be sent within a month after that. Is that true? I'm concerned that even if the GP writes a letter we won't get it in time. It took over 4 weeks for Dad to receive a copy of a letter from his GP records. It would be helpful to know whether this is true before seeing Dad's GP.

I'm so grateful to you for your support - I'm feeling less anxious about the visit to see Dad's GP with him.

If Dad's GP doesn't want to assist, is there any other avenue that could be explored?

Ideally you need the GP to comment on your father's ability to complete each of the PIP activities. I would expect them to be able to comment on some and not others, for example they may not be able to comment on his ability to prepare and cook a meal but may be able to say something about his ability to dress and undress if he has restricted movement in his upper body.

Once you have requested an MR, if you inform then that you will be sending additional information then you should be given four weeks to do this, but you should send it as soon as you can to avoid it passing another Decision Letter in the DWP's internal mail system.

If the GP won't provide support then you can access his medical records directly
