There is a template letter in the PIP section that you can use, you need to provide some basic reasons in the request and then go into more detail on anything that you attach.

Thank you so much for your help and support.
Thought I'd provide you with an update!
Dad was seen by GP on 22/12. The GP completed a thorough physical assessment. GP will complete a report for us to check. I provided the guidance that you've previously mentioned. Thank you once again. MR had to be sent same day (22/12) so the delivery was guaranteed by today (decision letter was dated 25/11).
Knowing that I wouldn't see the GP report (will be ready in January) before MR sent (nor would we have seen GP before MR deadline) I decided that it was best to focus on 2 activities (managing treatment and taking nutrition) with no points were scored (I could have chosen others with no points but MSE seemed to be their only consideration with these ones where the justification was "inconsistent with MSE, adequate concentration, adequate memory" therefore no points scored). There was limited time so I thought it wise to do the 'best possible' job on those (where I hope we should have the strongest case) and flag up some information that was sent but ignored (not listed on the front page) which may have had a bearing on the descriptors where memory and concentration factored in. I just hope I've done the right thing! Fingers crossed
When the GP report is ready to be sent - should it be sent with a copy of the MR too?
I couldn't find the template MR letter in the PIP section ... so used the template that was in the PIP Appeal Submissions guide on p2 (which may be the same one)?

It certainly needs a covering letter, make sure you add your fathers National Insurance Number and full name to each page and only send a copy, not the original.

The decision letter from DWP was dated 25/11. The MR was sent on 22/12 (sent special delivery so arrived 23/12). The GP is still working on his letter.
Based on these dates, when does the GP letter have to be with the DWP by? Is it within 4 weeks of DWP receiving the MR?

Based on the dates you have posted, your GP letter needs to be with the DWP no later 19 January but I would advise you to have it with them no later that the 12th if you can.
