At the questionnaire stage are we supposed to provide medical evidence?
I was asked on the phone for it, but other people haven't at this stage. What should I do?
I am expected to fill in form over Christmas when offices are closed and I can't get help. What should I do?

We recommend sending any medical evidence you have with the PIP2 when you return it, remember to only send copies.

Be careful not be caught out by the coming Bank Holidays when returning your form.
