i have recieved the medical report form in connection with my esa award though happy with my award as it was same as last time i am confused to a couple of things there in maybe you could help
evidence considered. the questionare,med 3.113. gp letter.copy of prescription,hospital corraspondence.gp medical record printout,and additiona limformation,letters from hospital.#
113 confirms restrictions in mobilising due to breathlessness.what is a 113, med 3 please reveiw in the medium term how long is this please x

An ESA113 is a short report from your GP, it's a standard form asking about your health, although there is room for a couple of specific questions.
A Med-3 is the technical name for a Fit Note, we regularly see it included in the list of documents, but it rarely exists, we think that the ESA software automatically adds it in and the assessors forget to remove it.
