Is the address for Subject Access for all info the DWP holds at Telford?
Do I write a seperate letter to PIP in Wolverhampton to ask them to send
to me all information they used to make their decisions? Is this also called a
Subject Access Request?

Requesting a Subject Access Request (SAR) :
SAR Form Download
From this page :
DWP Data Protection Policy
Or you can make the request in the form of a letter, as long as you make it clear that you are making a formal SAR under The Data Protection Act 1998.
Address the envelope : FAO The Data Controller, at the Benefits Office dealing with your PIP.
Write DPA 1998 SAR in front, top, left corner of envelope.
Once received, they have 40 calendar days to comply.
Sending Documents to the DWP.

I will write a letter to PIP office who issued
me with their decision, requesting copies of
all info they used to make their decision, excluding what I sent them. I have a feeling they have a report from a GP who was asked specifically not to share info with any third party till her report to ss & ot are redressed. I no longer live in that town.

Many thanks.
Question is, is it wise to send PIP a copy of email from my solicitor to show that the transfer of care for 19 hours per week was granted, till completion of a full assessment from ss & ot, but this was not done, because I moved away? The first two assessments were inaccurate, misleading, and full of lies. A third assessment was not completed. PIP downgraded from higher care rate (DLA) to standard care
