GP REPORT FOR PIP was created by shrinathji
Hello All,

My husband has received his PIP form for transferring from DLA to PIP. He had a life time award for mobility and care.
Our GP is going to write a medical report to support his claim. Our GP, also kindly wrote a medical report for my husbands ESA claim a couple of years ago and stated in his report that my husband should be in the support group. Fortunately by using your guides to fill out ESA my husband was put in the Support Group till July 2018.
Do you think he should put in this PIP report that in his opinion my husband should get the enhanced rate for daily living and mobility? His overall condition has really deteriorated and this is supported by his medical evidence.
I shall be grateful of any advise.

Regards
Radhika

Hi radhika, welcome back to the forum.

Being awarded enhanced rate of PIP depends entirely on scoring enough points against the PIP descriptors. Your doctor will do more good by describing how your husband's conditions meet the descriptors than giving his opinion about the level of the eventual award.

Many thanks for your reply.

However, just to be clear, for moving around, could my GP state that my husband can only walk less than 20 metres even with his walking stick and explain why?

Furthermore, when I called the office to state that my husband would like to claim PIP, the lady stated that she would be requesting my husbands old DLA form to be used in conjunction with the PIP application. To be absolutely honest, I did not think anything of this until I had put the phone down, as my husband was last assessed for DLA 15 years ago!!!!, so I am wondering as to why they would need his old form.
Would you by chance have any idea??? ;)

Yes, if your husband's GP states that he cannot walk 20 meters, and why, that should be helpful.

It is hard to say whether the DLA records will be helpful, it depends on whether your husband's condition has changed appreciably in the interim. It is entirely possible that they do not exist now anyway (as DWP destroy hard copies of records after a period of time).

Hello
one of the members named Susie had posted about 5 days ago, on the Forum that her GP had used the sample GP further medical evidence report form you give. Can you please confirm where I can find this, as our GP has agreed to write a detailed report for PIP.

Thank you
R
