Fortunately, I had sent copies of all GP/ Medical reports.

What you have stated does make sense.

However, I am just a bit worried that nothing has been missed.

Radhika,

Just go through the forms and the copies of all the documents you have sent and look the at the list of issues your hubby has mark ticks against them and if you feel you have missed anything off I am sure additional information can be sent just let the department know. Well I have received my PIP forms today dated 29 December got till 29 January to complete and send I can't say I'm looking forward to it good luck Radhika and try to relax easy said than done though I will keep my progress posted too

Hello,

I have a query. My GP wrote a detailed medical report for my husband and clearly outlined his diogenised problems and the problems that he is having with them on a daily basis.

However, last Monday morning we received a letter from Atos saying that they had written to one of the specialists listed on the PIP2 form for further medical evidence. I phoned them and they confirmed they had written to the GP.

I contacted the practice manager and she confirmed that they had received the form, but couldn't understand why, as the report that the GP had written confirms most of what is required on the form.

My husband medical problems go back 20 years after a serious head injury, 2 heart attacks and recently he has been diogenised with CFS and will start his treatment in the next 2 weeks.

My question is that on the further medical evidence form, it asks questions and states that GP should answer on his knowledge of my husband and medical records and to provide reports from consultants etc. I sent all the letters with pip2 form.

However, my husbands medical records do not state that he cannot prepare a cooked meal, dress and undress etc, or consultants records as no one really asks these questions at consultation. However, I did list all my husbands daily problems in December 2016 and gave the list to the practice manager and told her to put that in my husbands file.

My GP used this list, husbands medical records, Gps own knowledge and consultants letters to write detailed report in January 2017.

Can he state in the further medical evidence form that the patient has confirmed that they cannot prepare food due to their CFS?

GP is really busy and we were lucky he provided a detailed report, I am just worried that he may not put as much effort into this form. Can he just write on the form to refer to the detailed report sent????

You GP can respond in what ever manner they want, but I would expect them to complete the ATOS questions, as they are paid to do so.

The ATOS request is separate from the report he produced for you, although they can bring them together.

Thank you for your prompt reply.
Practice Manager has confirmed this evening that GP will complete form, but will be sending a copy of the detailed report that he had written for my husband and will also write on the form to refer to the report.

I also forgot to mention that when I called Atos to find out who they had written to, the person at the other end told me that my husband had been put down as a home visit. We had not requested one, so I am a little taken back and surprised with this.

Any ideas????

Hi Radhika,

In scheduling a home visit, it means that it has been acknowledged that your husband has significant problems.
