PIP (Invite from DLA) and GPs letter/s+Apologies

Happy New year and thank you
I am not comp literate + health conditions
so apologies:

My Q is with ESa if I recall a ' Pro Forma' form was sent to the GP to fill in.

What is the situation with the 'transfer from' DLA to PIP

1 s the GP sent a Pro Forma?for GP to complete ?
Or is it a straight written letter from GP?
My particular surgery 'does it seems not' do letters (generally)
I guess they can charge if so does that mean they will do a letter?

Is it expected that all 'transferees will be 'invited' by end of 2017?

If the DWP contact your GP and it is quite rare for them to do so in regard to PIP then it will be a GP Factual Report, there's a section towards the back of the PIP Claim guide that covers this.

I haven't seen any official update on the 2017 timeline, although I have seem comments about it being extended to 2018, our only reference point is the ESA transfer process which was due to finish in April 2014 and is still being carried out. My best guess is 2020.
