Apparently miracles do happen! Had a telephone call from the DWP re ESA to confirm my circumstances for satisfying the requirements for the Severe Disability Premium. Although no notification yet, it appears my MR request for PIP has resulted in being awarded the enhanced rate for PIP (daily living) - hence the ESA issue. It's unclear whether the standard rate of mobility has been awarded - I'll wait for the notification.
Once the notification has been received, I will post with more info and detail as to the approach that was taken in case it might help others.
In the meantime, I thought it might give one or two others some strength to know all is not lost with an initially negative decision and, although rare, it is possible to get a (mainly) positive outcome at MR stage.
The B & W guides were helpful but also thanks to Gordon for providing a starting point for case law (i.e. PIP help; LASA). His responses to a thread specifically related to Activity 1 (preparing food) were also very useful, even though he didn't entirely agree with some of my thoughts .
