Sorry Gordon but you are telling me to put no in box a and b if I can prepare food no problem and say so in comments box then you are saying if I can't prepare or cook my own food at all put no in 3b box ?? Is that correct ?

The question is do you need to use an aid to prepare a meal?

If you can cook a meal without problem (I know you can't) then you would reply "no" as you do not need to use an aid to cook a meal.

If you cannot cook a meal even with an aid then you reply "no" as using an aid will not help you to complete the activity.

You would only answer "yes" is using an aid allows you to cook a meal and you would not be able to do so without one.

The intention of the prompting, assistance or supervision is exactly the same. To be clear, they are not talking about someone else preparing and cooking the meal for you, this about you doing the work with someone else's help.

If you can cook a meal without problem the you do not need prompting, assistance or supervision so you would answer "no".

If you can't cook a meal even with prompting, assistance or supervision then you would also answer "mo"

You would only answer "yes" if the use of an aid or prompting, assistance or supervision would help you to complete an activity.

But as we have said before, If the tick boxes are confusing you then mark them as see below, but you must still answer all of the questions even if it is in your own words.

Hello Gordon down from the state of panic a bit now nearly finished assessment form my question is I have about 12 photocopies of consultants letters should I risk putting them in DWP envelope or best all together in my own with questionnaire in recorded delivery thanks

You should send them with the form. See

Sending Documents to the DWP, Maximus, ATOS or Capita

If everything will fit in the DWP envelope then use that otherwise your own. The Post office should charge you only the difference from what the DWP envelope pays for and the cost of a tracked post.
