hello, i am helping fill in the dla 2 pip form for my uncle, on mental health i wondered do you have to give the name of the person who helps him, how odten he helps him and for how long he helps him on all of the questions and is it best to write the details when filling in the boxes or is it best to just put the answers down after you have filled the boxes in. thanks

He doesn't have to be receiving care to receive and award, so you don't need to name someone but giving the care but it might be helpful if you do. You do need to explain why he needs the care, how often and for how long.
