To meet the criteria for

d. Cannot follow the route of an unfamiliar journey without another person, assistance dog or orientation aid. 10 points.

you are going to need to show that your mental health issues when you are out effect your cognitive ability to such an extent that you are unable follow (navigate) a route.

Yes, if you report a change then it will result in a full reassessment of your claim.

Ok. They say things like no evidence of cognitive problems in the past though.

So it wouldn't count if it was problems with extreme anxiety or psychosis for example?

The DWP's interpretation of Descriptors (d) and (f) has not changed this is still classed as an issue of following or navigating a route, you need to show that your anxiety or psychosis prevent you from being able to do this reliably and on the majority of days.

Ok. They already accept that I -

Need prompting to be able to undertake any journey to avoid overwhelming psychological distress to the claimant. (4 points.)

However they have not looked at that I would also need support, to do this reliably and on the majority of days.

So I suppose I need to point this out at the reassessment?

Yes, all of the PIP activities need to be completed in this way.

In my tribunal recently I made it 100% clear that I would not be able to complete an unfamiliar journey without the support and presence of another person to keep me safe.
I gave many examples and explained how I would get in a panic and make errors of judgement, like wanting to get out of a train when between stations, that could result in me putting myself in danger, another example was driving my car the wrong way around a one way system (!) and of course the basic symptoms of getting terrible asthma and panic attacks and incontinence in public.
I explained how journeys to an unfamiliar place was not a repeatable activity and that I could not reliably do it again the next day. I explained my best job ever I had to resign from after 3 months due to having to get off at every station on a train journey to hide in the toilet for 15 minutes before being able to continue, all so true, all of which has totally ruined my life both socially and career-wise, and yet it was all ignored.
I came away with the standard 4 points for mobility.
