If you are referring to
Q11b Do you find it difficult to mix with other people because of severe anxiety or distress?
if you answered "no" to this then you have said that he has no problems, however, this should be correctable.
Write a short letter explaining what has happened and explain what it should have been and why. Send a copy to the DWP office dealing with his claim and to the Assessment Provider as soon as you have an address, also, take a copy to the assessment and give it to the assessor and ask them to read it before they start the interview and include it in his file.

11a is about whether the claimant needs Social Support have a look at the guide to see what this means and whether your uncle needs it.
