ESA home assessment

I sent my esa form back before xmas having previously been awarded 3 years in the support group for ME, orthostatic intolerance and agoraphobia/social phobia. I did fill my form in in great detail and stated I would not be able to attend an assessment centre and would need a home visit. However today I got my appointment for the assessment centre at the end of this month. All sense has left my brain as I did not expect this and do not know what to do. I was wondering if anyone could advise me of how to tackle getting a home visit. I do feel like I will lose my esa this time as there is no way I can attend this or the job centre. I know things were done in a certain way with atos but I know have maximus I think (letter from Bootle)

You need your GP to write a letter explaining why you cannot attend at their offices.
