DLA to PIP was created by Leeann

Hi, my husband has just got a letter telling him his dla is coming to an end and that he must ring this number before the 15th. Is this just so they can get information to send the pip form out or are they going to ask question about his health etc. I just want to be prepared before I ring. Grateful for any help guys
