It states that I may or may not be invited for medical assessment also. I'm not sure over the phone is a good thing for me, straight to form would be less of an anxiety issue. Advice and thoughts much appreciated.

The form usually arrives 5-10 days after the call. You will have four weeks to complete it although this also includes postage time at both ends. You can requests a two week extension if you need to.
