Near the end of the MR there is a part that tells you about SSCS1 and how long you have to reply. Like Gordon has mentioned, you should get copies of medical records and any other evidence you have to support your claim and "photocopy" and send it away. The court will reply back to you and let you know that they have received the paperwork you have submitted pretty quickly (within 2 weeks).
