You need to give a reason for the MR request, there is a template letter in the PIP section that you can use.

Also, include a date when you would expect to have sent the additional information by, the DWP usually allow four weeks.

Thank you Gordon,

1. Ok when I Send in the MR, when it comes to the EACH DISPUTED ACTIVITY IN DETAIL.
I Will try and add new information/Evidence, should I Still refer to my original PIP form that I Sent highlighting existing evidence I Had already given just in case they missed/Overlooked the initial claim.

2. Also do I Need to send back in the original form that I Initially claimed for ? I Presume the original form I Sent will be looked at and reviewed.

3 is it ok as in Q1 preparing food to say you think both 1e or 1f could apply or just concentrate on one ? [ I Have tried to assist him to cook but ends in food ruined]

4. is making a sandwich classed as Q1 Preparing food.

If you think that the information in and with your PIP2 explains your problems then there is no problem with you referring to it in your MR request, in fact it can be an advantage as Decision Makers often take the view that the first information they receive from a claimant is going to be the most accurate.

You don't need to re-send anything that you have previously supplied as part of your claim it should all be in your file.

There's no problem with your referring to multiple Descriptors, in the case you mention they have different criteria both of which you could reasonably meet.

Making a sandwich does not qualify as a simple meal, however the activities required to make a sandwich are (to some extent anyway) ones that someone would need to perform to at least prepare a meal if not cook one, so be aware of this.

hi hope you dont mind me msg you , like your husband i suffer from severe mental health problems and i was also awarded the same points in my assesment report ,, i sent a writen request for a m.r decision with more evidence and it still came back the same decision ,,so dont feel disheartend if this happens to your husband ,this seems to be quite normal with m.r requests , when you write the mr , refer to criterias in which you have medical evidence to follow it up if possible as that may help them come to a different decision .failing that if it doesnt change you have the option to appeal ,,this is what i did ,it took nearly 12 mths from first sending off for pip to the actual appeal but i was lucky i did get a award both standard for daily and mobility ,,so my advice would be to you ,dont give up even though its very stressfull and time consuming its worth fighting for what your husband is entitled too , i was dreading going to appeal as ime agrophobic as well ,but decided to go for a oral appeal as i needed them to see the real me and felt if i hadnt of gone they wouldnt of seen how overwhelming pycologicaly distressed i was being there , remember if it does come to a appeal these people are independant and actualy treat you like a human being with respect not just a ref number like the dwp do ,,realy hope your hubby m.r is succesfull , wishing you good luck ,

I got my claim rejected and was told it was unsuccessful due to not being able to attend my assessment despite the fact they ignored my request for a home visit on grounds on physical and mental health grounds. I read on here to get your MP involved. It was the next best thing I did after joining this site! They have their own contacts within DWP who they contact on your behalf. My strong advice is to email your MP's office and sign in both names ( as they need 2 constituents to act for them)
My MP's office appointed me a worker from within their office who then spoke to me and their contact until we got a successful result - and all back pay! That is what they are there for. And they are a "somebody" asking questions on your behalf whereas I believe they see us as numbers and not people!! WRITE NOW asking for a mandatory reconsideration and send a copy of that to your MP as it will save you explaining it all twice!! Also make sure you put in your letter that you will not ask for your reconsideration to be cancelled unless they receive it in writing from you. Read the guide tis site supplies on making a mandatory reconsideration as makes writing the letter so much easier - and enclose any further evidence if you have any. Good Luck x
