Pip mobility question

Another question..My assessment is on the 28th and I've just received another letter from my neurologist that I'd like to use as further evidence.
Do I send a copy to Atos or the DWP? ..or take it in on the day?

If you think it significant enough to allow a paper assessment then send it now, if not then take it with you to the assessment and ask the assessor to read and include it in your file before they continue. They are required to do this.
I would also send a copy to the DWP, marked for the Decision Maker to the office dealing with your claim, the address will be on the PIP letters that you have had from the DWP.

I'm still not clear regarding the "planning and following a journey" question.
I'm not able to go out alone at all due to anxiety about being around people and being outside, my partner has to be with me and even then it's only possible a couple of times a week...sometimes I'm unable to go out for weeks at a time.
I've been this way for years and I've explained this on my previous pip forms but I've only ever received 4 points for needing prompting.
I know the rules have changed recently for people who need prompting or support during the journey and not just prompting to actually leave the house ( I have panic attacks and my partner helps to keep me calm and gets me somewhere safe..stops me from just turning around and going home )
I'm not sure what points I should receive , I've read through the guide but I'm still not sure if any of the descriptors apply to me? Except for "needs prompting" ...but I need someone with me...not just prompting to get me out of the door?

Based on what you have posted, that your problems are to do with anxiety and going out then you appear to be assessing yourself as meeting
e. Cannot undertake any journey because it would cause overwhelming psychological distress to the claimant. 10 points.
You will have to show that your anxiety is extreme and meets the phrase "overwhelming psychological distress".

I remember reading one of the guides and it saying something along the lines of..typically this person wouldn't have left the house for years..very unlikely to attend their assessment..
Has this changed?

I can only go by what you have posted. Descriptors (d) and (f) are classed as being connection with navigating a route, your post does not suggest that you would have any cognitive problem that would prevent you from doing this.
