If the guide says that the descriptor " cannot undertake any journey" only applies to those who typically haven't left their house for years and would be unlikely to attend their assessment...I'm not sure how that could apply to me ? as I do go out..admittedly rarely and never alone , I need my partners support.
I don't think descriptors d or e apply to me...but I need more than prompting just to leave the house...I need someone with me.

I know that is what the guide says, but you may be able to argue by reference to reliably and on the majority of days that "cannot" does not mean "never". Based on what you have posted this is the only way that you will score more than 4 points. The Descriptors don't take account of your needing more than prompting, you will only qualify for more points by showing that you meet the specific criteria for them.

Do they have to read them? Because she kept saying " I don't know anything unless you tell me"
Then she asked for a list of my medications and I told her I had already sent in my prescription list...she said she hadn't seen it..although she eventually found it on her computer.
When she asked me if I had any problems getting on and off the toilet , I said I didn't..which is true. But later on I mentioned about being incontinent and she got annoyed and said " see that's exactly what I'm talking about..I don't know anything unless you tell me".." now I'll have to change my report"
Does it sound like she didn't read my form and evidence? Because now I'm worried about what she may not have put in her report.
Also when I was leaving she said " you'll know better next time" ??
I also had to sign something before I left , which I hadn't had to do before. Any idea what that was?

They are required by the DWP to read the form and anything attached to it before they carry out the assessment. The reality is that most don't!
Give it a couple more days and you can request a copy of the assessment report by contacting the DWP office dealing with your claim.
There shouldn't be any requirement for you to sign anything unless you had asked to record the assessment or were claiming back travel expenses.

Now I'm worried what I signed?
I didn't ask for the assessment to be recorded or claim travel expenses.
She just said I had to sign this form so she could send off her report?

The only other thing I can think of is that they want to contact your GP and this is why your signature was required.
