Correct form ? PIP1003

The 1003 form is the PIP2, the latest PIP Claim guide covers this.

Sorry but from your description I cannot tell you, here is a link to a PIP 1003 form to see if your form looks similar, you may want to check with DWP that you have been sent the correct form.
