It's very unlikely that they will ring you at all, but they certainly won't until they have received the extra information.
I would not mention that you intend to appeal, it can restrict the options that the DWP believe they have in offering an award.

The PA4 Consultation report gives the type of professional as Nurse, what type of nurse do the face to face ?

should I Phone as well as post, or post be ok ? I Will be posting Special delivery so can track letter.

They could be any kind of nurse. You may be able to look them up.

Personally I would ring a couple of days after the form is delivered to make sure that they have it, despite the tracked post.
