To be clear, she's a nurse so they are qualified they don't just don't have any additional qualifications.
All assessors have to pass a qualification examination set by the DWP.

Should I ask for part or full MEDICAL HEALTH RECORDS from my husband’s G.P. Practice [he is in his early Fifties] ?
Is it possible to receive copies or information as regards him been on the SEVERE MENTAL ILLNESS REGISTER ,
Would letters from family or friends help in that they can back up which descriptors we think apply?
Any other suggestions would be welcome,

You can requests a copy of his medical records from his GP surgery
Requesting your medical records
Charging for medical records
Family and friend scan make a statements of the problems that he has that they are aware of.
They need to write a letter explaining who they are, how they know him and how often they see him. What his problems are and how they see him deal with them. Lastly they need to attest that the information they have provided is true and accurate and sign the letter.

The copy of the face assessment had a number of errors, one error was that my husband does not use anti-depressants when he does and this is backed up by his Doctor.
Should I explain this by saying something like as this was incorrect then the face to face cannot be used with any accuracy?
I’m trying to find the relevant words to say to the decision maker at the Mandatory reconsideration that the PA4 Consultation report is badly incorrect even to the extent of saying he is not on any anti- Depressants

I would certainly mention the mis-reporting of the anti-depressants, this should be easy to show as incorrect from his prescription repeat.
Is it enough to make the report redundant, I'm afraid that I think this to be unlikely, although it is an important issue it's not what I would call a fundamental problem.

My husband is in his fifties so no idea how many pages we will receive, Should we [a] send only copies and not originals, send full report, [c] send full report and highlight relevant areas/evidence to back up appeal or [d] send only relevant pages relevant to appeal ?
