Supplying evidence with ESA50 Forms

I received the ESA50 form yesterday. I am on High rate care & Mobility. Most of my paperwork proof is from 10years ago when I was tested for deafness/vision and after my stroke left side weakness. I received help from occupational team. I did however move out of area, although I was seen by occupational therapist in new area I did not receive any new paperwork exept for a stair lift they wanted to put in for me. They only mentioned verbally about lighting and hearing wise I've been checked out and helped by local hospital. As the paperwork is old and conditions still ongoing should I still put copies of these 10year old letters with the esa50 form? If not what should I do in order to prove the vision/deafness/ left side weakness problems? Thank you for any help.

Providing the evidence is still reflective of your conditions then you can use, just explain why it is still relevant on the form.
