Made a mess of my Pip form was created by ethan
Concentration was bad and eye to hand contact was out of synch so the form is riddled with crossed out words and alterations. I may even have responded incorrectly and at times incomprehensibly to questions that are not applicable etc. Is it too late to request another form as its due for submission next week? I was wondering if could tipex everything out and photocopy the blanked out sheets or simply explain the problem and submit it anyway?

I would certainly ask for an extension to the time to return the form.

You can ask for a new form at the same time.

If you have a printer you can type out your answers and attach them to the form, just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.

Attach the pages to the back of the PIP2 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

There could be an advantage to using the original form and simply crossing out your answers and referring those assessing you to the extra pages.

If you have to use the existing form then an alternative to Tippex would to stick a blank piece of paper over the comments box.

You can ask for a second extension, but it has to be decided upon by a Decision Maker so usually takes 4 or 5 days to be confirmed.
