There are two problems with this.

As the DLA was awarded in regard the Epilepsy you risk everything you have already written in regard to Awareness of Hazard being ignored by the DWP because you have linked her problems with this activity to a physical condition.

Also, the activity is not about forgetfulness, it's about not understand the danger in the first place, If she understands the risk of leaving something on a lit hob but then forgets that she has done this then I would not expect her to score points.

Yes that the problem as I feel its a mixture of everything

from a child she has always been forgetful and absent minded gets confused easily.

Now whether that was because of her epilepsy or the polymicrogyria I do not know

I don't know if the polymicrgyria is classed as cognitive or intellectual problems

The neurologist just diagnosed her with this All he said is she has epilepsy

An then said she had excessive grey matter in the right hand side of her brain I think the frontal lobe and basically happened in the womb he said it affects her thought process and ability to process information so she gets things mixed up or the signals just don't get through properly I am not sure, when the neurons are forming on the brain they don't land in the right place.

It can cause Mental retardation of different degrees and can also cause cerebal palsy in extreme cases
it can also cause fits so now I am no wiser and baffled
She is just very forgetful and absent minded and it can affect motor skills

But she is also very depressed and has Borderline personality disorder
And all her thoughts are dominated by her past and present circumstances with regards to her son.

She has left cooker rings on a few times burnt food her hoover was smoking earlier and she didn't notice as she also has poor eyesight

she forgets lock her doors has left hairdryer on the floor and her son got hold of it and burnt 3 rings in carpet I don't know if this is because one condition or another
She has just been with her sister and our grandson and as they were going he went in road and she didn't notice until my mrs said she is not aware most of the time about risks and dangers and what it is I have no idea this form is a joke because you have to link it to physical or mental

When she blows up into a rage with her BPD she cant stop herself and is unpredictable and erratic can throw things like cups at wall punch doors scream shout storm off and at times like these she is not aware how she is being I have never done a harder form than this as I did my Mrs PIP form without help they gave her standard on both for 10 years and then a mandatory reconsideration and got enhanced on both without appeal

This form is the hardest I ever had to do as my daughter can cope and cant deal with it doesn't like admitting her problems and she is in a complete mess most of the time due to her circumstances and being very low

I have to get it back to them by Thursday and Doctor only doing a letter for Monday as wee only have limited paperwork for evidence from neurologist and mental health from when she was under them therapy as just ended in January

Is the Going out for people with agrophobia and learning difficulties not depression and BPD which causes her to be manic at times and not think clearly and fearful of going out as she has Mixed anxiety and depressive diorder

It's about being unable to go out without having someone with you due to mental health issues, so agoraphobia is an obvious choice, but any MH condition that limits her ability to go out without supervision is relevant.
