I would assume that the 'DA' stands for Disability Assessor rather than a specific branch of nursing.

I assume Disability Assessment Nurse, if you have their registration number you can look them up on
www.nmc.org.uk/registration/search-the-register/

HCP's do not always include their Registration Numbers on Assessment Forms.
But if it is asked for at the assessment under code 23.5 of the NMC code of practice nursse are required to provide their PIN (reg. mumber) if it is requested.
So if HCP is a Nurse you can request their PIn when they introduce themselves and make a note of it for future reference.
