Any experience with HMRC R40 form?

Hi, I wonder if anyone can help with my confusion on filling in an R40 form.
Apologies if not a benefits question but does relate to my benefits as in what to put on form.
Back end of 2014 and also beginning of 2015 i received bank refunds for charges previously paid.
Even though it stated on both letters i could possibly claim back the 20% tax they automatically took off the 8% statutory interest compensation i didn't realise until someone just mentioned it to me that i may fall into that category to claim it back from then.

I received then and still do Income related Esa support group and also DLA high rate mobility, Low rate care.
I am very confused as to if i am eligible to claim due to tax etc.
I vaguely remember getting a P60U when i was awarded ESA support group on transfer from IB and from advice i got here i then filled in information to be moved on to the income related ESA which i was awarded. Not sure at which point i got the P60U.
I will be digging all my information out but basically as it says i should include p60's with claim ( i don't think i get them) i am getting more and more confused as to what is taxable, if i actually can claim it back with the benefits i am on, and should DLA be put on form also? it's all getting more confusing the more i try to look up answers so wondered if anyone has any advice on this?

I'm sorry but we can't help you with the completion of this form, try and get face to face advice, the CAB may have experience in this area.

I can tell you that both DLA and Income Related ESA are not liable to tax.

Thanks for the quick response. No problem, wasn't sure if you would of been able to help with this.
Actually, it's a great help you giving the information to me DLA and ESA IR are not liable to tax.
