My PIP2 FORM is different to the self-test!

Hi everyone (glad you are here!)

I'm yet another disabled person on high-rate DLA who is being encouraged to enter into the dreaded PIP application process. :(

I phoned up by the date given on the DWP letter I received and was asked some questions ( to enable them to fill in the PIP1 FORM I presume).

Shortly afterwards I received a PIP 2 FORM through the post. Before looking at it I took the liberty of doing the Benefits and Work 'self-test' available on the website. Doing it for the first time with a minimum of preparation I got a score of 20 for daily living and 8 for mobility, but the problems started when I looked in the actual 'Personal Independence Payment-how the disability effects you' PIP2 FORM I've been sent.
Its different!
Its similar, but the numbers of the questions, the wording and the way its set out is different. An example is question 4 which the self-test says is about washing and bathing, yet my PIP2 FORM says question 4 is 'Eating and Drinking'.

Its fair to say I am a little confused, am I taking the self-test too literally or have they updated the PIP2 FORM? (e.g. is the self-test out of date?)
The date on my PIP2 FORM is 14th March and it has to be in by 14th April.

Any responses/ help or assistance would be greatly appreciated!! :cheer:

It's a simple mistake :)

The test has questions from 1-12, the form has two extra initial questions, so question 1 on the test corresponds to question 3 on the form.
