A few questions for when PIP form arives

I haven’t had my PIP assessment form yet but am waiting for it these are a few questions I would like advice on when the time comes to fill in the form.

At the moment my wife is unable to wash or dress me as she has had major back surgery, I have social services “homecare “coming in the morning to shower dress me & make my breakfast, the only part of my body that they do not wash are my front genitals everywhere else is washed should I say on the pip form I am unable to wash any part of my body?

The home care also help dress me upper and lower clothing they tuck in my shirt the only thing I do is my trouser button and zip, so again would this class as not being able to dress at all on the pip form or needs assistant to dress top and bottom?

I have a K9 Partner dog which has been trained to undress me, partially by the charity and the rest done at home by me so would this class as having assistants for undressing?

I use an electric wheelchair, as my mobility is very limited, my physio has advised me to only transfer from my wheel chair to a seat, bed etc. with the use of a crutch, if and when I get my assessment the assessor asks me to walk with the crutch, can i refuse for my own safety reasons?

I'm taking your post as written :)

You won't score points for things that you do not do only those that you cannot do, so it is not the aid that you receive for washing and dressing that will be looked at, but the aid that you actually need that will be considered.

So if you are to score for these activities you must explain the problems you have with doing these things yourself, what are the conditions, how they affect you and why you cannot use aids to help you do them on your own.

You can refuse to carry out any activity that would cause you pain or distress but you will need to explain to the assessor why you are refusing.

OK thank you for your quick reply just need to go into more detail why I need help in carrying out these tasks, Medical letters etc would help?
