I applied for ESA in july when i was diagnosed with breast cancer, Previously I was on JSA as I was made redundant, i had only been signing on for 2 months when i was diagnosnosed. I also have osteo of the spine ans asthma.

With the help of macmillan WRO I was excempt after sending in aESA50 no medical i was put into support group in SEPT 09. Then i receive another ESA 50 due in in Jan 21, my treatment for radiation have finished. The sick notes in the past said BC.I am dreading filling in the new form as i am no longer exempt and it all depends on what i say on the form.

Two surgeries and lymph nodes removal has left me worse phsically and mentally. The radiation tratment is still causing me fatigue. will these count on ESA 50, or just concentrate on osteo and asthma as i am aware symptoms and side effects may be overlooked.I am still on medication for BC which causes further side effects.

It is recommended that you give full details of all your medical conditions and any associated after/side effects of treatment.

Atos tell claimants not to send any additional (medical) evidence with the ESA50, but the advice is to send whatever evidence you consider appropriate as it has to be taken into account. Send a copy of the (medical) evidence to the DWP, as this also has to be considered by the ESA decision maker.

Ensure that you record your name and national insurance number on any additional sheets that you send with the ESA50.

Hi

I am filling in my 2nd ESA50. I had my first medical in July 08 and it looks like i may require another. My question is should i send ATOS a copy of my DLA award stating that i am virtually unable to walk along with an OT report. The doctor at my last medical stated i could walk over 200 metres although i attended in a wheelchair. Mt OT letter dated May 08 states i can walk 8 metres.

Will ATOS look at this info at all.
