Hi - with reference to completing the IB50 and not enclosing documents - could I ask: As the convering letter with the IB50 form asks not to enclose additional documents with the form when returning it - I am completing the IB50 for my review and I have printed out the additional information sheets to use again from your guide to making a claim on mental health grounds and which I have previously stapled to the IB50 form when returning it - could anyone tell me if it would still be ok to do as ,to me, they form part of the form and are not "additional information or supporting documents as such. As I am not a regular poster I hope I have posted this correctly and in the right place - I apologise if not!.

Hi,

Yes you are in the right place, albeit if you use the search facility you will find your question has been asked and answered a number of times.

Re your IB50, although it has nice little boxes for claimants answers, not everyones answers will fit in the small box on the form, particularly if the answers are complex or there are multiple illnesses etc.

Thus in order to send in all your medical evidence there is no option other than to use continuation sheets containing your evidence.

Just remember to add your name and national insurance number to every sheet and retain a copy.
